Perhaps he has the same disease Joseph Merrick (the Elephant Man) had: neurofibromatosis or Proteus Syndrome? Possibly?

Looks natural like a disease, unfortunate I wonder if plastic surgery can do anything to help. But would probably be expensive.

Sorry lakelady, you got it completely wrong. It has been accepted that The Elephant Man was actually suffering from Proteus Syndrome and not NFM, which is likely what afflicts this beggar.
